Here is a breakdown of how Necrotic Plague currently works:
-It deals Shadowfrost damage and scales with both Frost and Unholy's respective Masteries.
-It ticks once every 2 seconds. Each time it ticks, it gains a stack.
-It can stack up to 15 maximum stacks and lasts 30 seconds.
-The only ability in the game that can extend the duration of Necrotic Plague is Festering Strike.
-It cannot overwrite itself.
Necrotic Plague spreads in two ways
-It has a jump mechanic. When it "jumps", it copies it current duration and stack count to the new target. Therefore, if it jumps at 7 stacks and 15 seconds remaining, it will begin its life cycle on the new target at that same stack count and duration.
-If you apply NP to a large pack of mobs and let it spread "naturally", it will jump off each target. Therefore, your infection order would go: 1-2-4-8-16, etc. Each time it ticks, it can jump to any available target.
-Pestilence can also spread Necrotic Plague. This means that it spreads Necrotic Plague to targets that have not been infected with it yet.
-When Pestilence spreads Necrotic Plague, it resets its duration and stack count on new targets. This means that they stat from 1 stack/30 second duration.
-If Pestilence hits any targets currently infected with Necrotic Plague, it adds a stack to the disease. However, Celestalon has said on Twitter that this is unintended. Until he says otherwise, assume it is a bug.
-Death Coil and Frost Strike, when talented into Plaguebearer, will add stacks to existing Necrotic Plague infections. They will not in any way increase its duration.
I hope this was helpful.
